On the operational level the number one accessory in male dress is a name tag.  It can say, "Hello," "Let's talk," or "Let's do business." A name badge is to be used to help others know you by your name. It can be used to facilitate an introduction. It may contain a first name only, (for non-executives,) last name only (for a person in uniform), or a first and last name, (for an executive or for each visitor to your organization.)
A business card holder, or a high quality "Stick on," badge can be worn to keep each person informed of each name.
The name of a person is to be listed without a social title such as Mr., or Mrs. A prefix title Miss could be added. A nickname (in parenthesis,) following a first name may be added. It is to be excluded from a business work card. It might be marked with a full name and professional titles, such as M.D., placed next to it with or without a position title placed beneath and center it. Adding this could be nice. Any academic title such as Ph.D. or LL.D. is to be avoided. (Very few observe this rule.) The name is to be in a size large enough to see from a distance.

The name for an officer Company Grade or above is to be listed in signature block form: rank, and then name. The name for an officer below Company Grade is to be listed in nameplate form: Name, and then rank.

When a name badge contains a name only, it is to be placed high on the right lapel, just below the shoulder bone, or attached to or just below the shirt collar. It is to be placed up high next to a left lapel, when it contains the name of a company and a person. It can be pinned to a jacket or a shirt. It may have a piece of cardboard, or a magnet, placed behind it.
A name badge is to be worn by each person in the room except by the person to whom everyone in the room owes allegiance.  It can be worn by everyone.
It may be worn at the collar to show courtesy, above the pocket to show authority, at the chest to show security, or at the belt to show adherence to a condition of employment. This placement mitigates the primary function of a name/identification badge, and makes it more of a pass. There are to be limits to how low one could have to go to learn your name.
A name tag can be worn at all times during working hours. When visiting your name badge can be replaced with one provided by a host. It may be issued and provided at no cost. It could be replaced at a cost to the badge holder. It might be supplemented by a name placed on a desk, door, or a building.
